Get ready for your upcoming appointment!


  • Have you completed your Patient History Form? Click on the link to open.
  • Need any refills on any prescription medications? Fill out our Drug Request Form here.
  • Is your dog on a special diet that you need more of? Fill out our Food Request Form here.
  • For puppy/kitten vaccinations and annual exams, don't forget to bring a fecal sample! Please bring a fresh sample (passed within 24 hours is best)


Is it your pet's first time visiting us at Mililani Town Center Pet Clinic?

  • Don't forget to email us your pet's previous medical history. Please send all records to mtcpetclinic@gmail.com
  • Knowing your pet's medical history is the most important part of ensuring that we give your pet the best care possible.
  • We can advise you on whether your pet should be updated on vaccinations, or possibly receive a vaccine that was not previously required.
  • Previous health conditions may determine how the doctors proceed with treatments
  • If your pet doesn't like coming to the vet, this allows us to prepare to make your pet's exam as stress free as possible.


Bringing in a stray, feral, or rescued pet?

  • Microchipped animals must be registered to you as the owner in order to receive any type of treatment.
  • Due to Hawaii state laws that issue ownership of the pet to the person whom the microchip is registered to. If the pet's microchip is not registered to you, please contact our office for further assistance.
